Dear Parents

As part of our systems around protecting our pupils, their families and our staff as regards COVID-19, we are asking all parents to complete a “Return to School COVID Form” confirming individual’s circumstances as regards symptoms, close contact and quarantine.

This form must be completed for each pupil who attends the school.  Where forms are not returned, pupils will NOT be permitted to return to school.  All forms should be completed and returned by Monday 10th August 2020.

In order to prevent the spread of COVID-19 in the school and to protect those who may be vulnerable, this form must be completed accurately.  If you feel that we should be aware of any individual circumstances please let us know.

This information will be used as a base-line for Track & Trace within the school.  If any pupil’s circumstances change the parent must immediately notify the school.  Any pupil displaying symptoms or who has been in close contact with a confirmed or suspected case, who has been advised to self-isolate or who is quarantining must NOT attend school.  Please bear this in mind as regards travel plans going forward.
